Earn the activity patch: To earn the museum activity patch, visit the museum
and complete the worksheet.  Have a museum employee review your completed worksheet and sign the form below to certify fulfillment of the patch requirements.

You can download the worksheet or pick it up at the museum.  You can either
schedule a guided tour, or tour the museum on your own.
